If you’ve ever looked into therapy for anxiety, you’ve likely seen the acronym CBT. Short for Cognitive Behavioural Therapy, it is the most researched and effective form of talk therapy in the world.
In Ontario, CBT is the backbone of our public and private mental health systems. But what actually happens during a session, and how does it help “fix” anxiety?
The Core Concept: The Cognitive Triangle
CBT operates on a simple but profound premise: Your thoughts, feelings, and behaviors are all interconnected. When you are anxious, these three elements form a “vicious cycle.”
- Thoughts: “I’m going to fail this presentation.”
- Feelings: Racing heart, sweating, intense dread (Anxiety).
- Behaviors: Avoiding the presentation or over-preparing until 3 AM.
CBT teaches you that while you can’t always control how you feel, you can change how you think and how you act. When you shift a thought or a behavior, the feeling eventually follows.
Key Techniques You’ll Learn
CBT is not “just talking.” It is a skills-based approach where you act as a scientist, testing out your own beliefs.
Cognitive Restructuring (Thought Challenging)
Anxiety often relies on “cognitive distortions,” mental shortcuts like Catastrophizing (assuming the worst) or All-or-Nothing Thinking. Your therapist will help you use a Thought Record to catch these distortions and replace them with more balanced, realistic perspectives.
Exposure Therapy
Anxiety grows when we avoid things. Exposure therapy involves gradually and safely facing the situations you fear. By doing this, your brain learns a vital lesson: the thing you feared didn’t happen, or if it did, you were able to handle it.
Behavioral Activation
When anxiety makes your world feel small, Behavioral Activation helps you re-engage with activities that bring you joy or a sense of accomplishment, effectively “starving” the anxiety of its power.

Because CBT is so effective, the Ontario government has invested heavily in making it accessible.
- Ontario Structured Psychotherapy (OSP) Program: Provides free CBT for depression and anxiety-related concerns. It includes self-led resources with coach support or individual/group therapy.
